Hi,
Maybe it is off the screen somewhere. Try setting your screen to a higher resolution; see if it shows up to the right or lower down. If you are brave you could temporarily rename the HKEY_CURRENT_USER\Software\Microsoft\Office\12.0\Access\Settings key to see if that has any impact.
